    Community Health Worker

    The Community Health Worker (CHW) Program connects individuals and families with trusted, community-based support to navigate health care, social services, and everyday challenges. CHWs build relationships, remove barriers, and empower people to improve their well-being and access the resources they need.

    At Wesley Family Services, our Community Health Workers (CHWs) serve as trusted members of the community who help bridge the gap between individuals and the resources they need to live healthier, more connected lives.

    CHWs work one-on-one with individuals and families to identify their needs and connect them with services, including:

    • Access to health care and insurance

    • Housing and transportation assistance

    • Food security and benefits navigation

    • Employment and education resources

    • Behavioral health and substance use support

    Whether you’re navigating a complex system or simply need someone who understands your situation, our CHWs are here to listen, support, and guide you every step of the way.

    What Does a Community Health Worker Do?

    Our CHWs are trained professionals who:

    • Build trusting relationships with individuals and families

    • Provide culturally sensitive health education and outreach

    • Advocate for clients within health care and social service systems

    • Offer follow-up care to ensure services are received and barriers are removed

    • Help people gain skills and confidence to improve their well-being

    Why It Matters

    Community Health Workers are often from the neighborhoods they serve. This shared experience fosters trust and makes it easier to:

    • Understand the unique challenges individuals face

    • Provide personalized, non-judgmental support

    • Promote healthier communities from within

    Eligibility Requirements
    • Our normal program hours are Monday through Friday, 9 AM to 5 PM. For non-emergency needs, evening and weekend appointments are also available.

    • Available to Allegheny County residents only.
